Having considered the documents registered in the relevant establishment records, it is hereby resolved that the above complies with the provisions of the aforementioned regulations and therefore, authorisation is granted.

To authorise the operation of the above referred establishment with the following elements:
- Health Units: General/Family Medicine, Oncology
In accordance to article 14th of Decree 69/2008, in the cases of authorisation of operation and modification, the date the authorisation is granted is deemed to be the date of commencement of the effective term established in the aforementioned decree for authorisation, at the end of which authorisation must be renewed in the cases provided for in sections 1 and 2 of the said article, by means of filing an application within three months prior to the date of termination of the aforementioned term.
In the same way, in accordance to articles 13 and 15 of the same regulation, the authorisation granted shall be terminated for the establishment or any of its services, should its activities be interrupted for more than six months. Authorisation shall become invalid, should the conditions giving rise to its granting be altered substantially. In the same way, this authorisation is granted subject to any others that may become necessary in application of the current regulations.
All techniques, technologies and procedures to be carried out in the health units authorised in the establishment must be carried out by health professionals with the relevant qualifications and skills for the purpose. These professionals must comply with the safety and cleanliness conditions required and follow the specific norms and procedures applicable.
